Do you just hand over a report, or do the work?

Either. Some clients want the fixes implemented end-to-end; others want a prioritized spec their own team can execute. The audit is the same rigor regardless.

How is this different from a plugin's SEO score?

A plugin checks a page in isolation. A technical audit looks at how the whole site is crawled and indexed — the issues that plugins can't see are usually the ones holding rankings back.
